Lincolnshire Partnership NHS Foundation Trust (LPFT) has been nominated in the 2024 Lincolnshire Care Awards, which aims to recognise those on the front line of social care in the county.
Led by Lincolnshire Care Association, nominations are encouraged from service users and their families, and LPFT has been lucky enough to pick up four nominations.
Our Every Voice Patient Partners, Building Together Focus Group and Community Mental Health Transformation Team have all been nominated in the ‘Co-Producing Together’ category.
The award, jointly supported by the It’s All About People programme (an online resource for Lincolnshire health, care and wellbeing staff) and Every-One (a Lincolnshire-based charity that supports unpaid carers and the people they care for), recognises teams, services or providers that can demonstrate effective co-production with people and those who support them. The judges in this category are looking for those who understand what true co-production means and can show effective change developed with people through valuing their insight and input from the start.
The Building Together Focus Group was nominated for its ongoing work supporting the design of our new acute wards in Lincoln and Boston. The group works together with our clinical staff to equally ensure the patient and carer voice is heard and to adopt a patient centred approach to shape the design and development.
The Every Voice Patient Partners were nominated for the various projects they have supported to date, not least the recent People Awards.
The Lincolnshire Community Mental Health and Wellbeing Transformation Programme team was nominated for its collaborative work with partners across primary and secondary care, local authorities, Voluntary Community, Faith and Social Enterprises (VCFSEs) and local communities to co-produce innovative mental health services. They have also launched the co-produced the H.A.Y. Lincolnshire website. Find out more about the Adult Community Mental Health Transformation Programme.
The final nomination goes to Liz Holmes, Assistant Practitioner in the Older Adults Community Mental Health Team in Grantham, who is nominated under the ‘Outstanding Partner’ category. This award recognises exceptional people who are dedicated to providing quality products/and or services to the sector.
Liz works with patients in her local care homes, providing support and advice for those patients with dementia, monitoring medications and advising care homes on non-pharmacological approaches to support patients who may be distressed on arrival.
The award evening takes place on Thursday 27 March 2025 and we wish all of our nominees the best of luck.
