Community groups and businesses are being invited to get involved in a conversation about how everyone can work together to help those who need support.
Integrated Neighbourhood Working in Stamford is already having a massive impact by empowering people to overcome issues such as loneliness by supporting them to improve their health and feel happier in their community.
An event being held by Lincolnshire Community and Voluntary Service aims to celebrate those successes and look at how different conversations between patients and professionals are leading to better results for those who need support.
Lisa Gibson, Social Prescribing Lead, said: “The event will be a chance to look at what Integrated Neighbourhood Working has already achieved and how we can continue to develop in a way that values the voices of patients and puts them at the centre of their own care.
“This event falls within National Conversation Week and having conversations is a key part of Integrated Neighbourhood Working.
“It is about asking people ‘what matters to you?’ rather than ‘what is the matter with you?’ and working with them to find solutions to the challenges they face and the issues they want to overcome.
“Everyone has a role to play in this joined-up approach and we are hoping as many people from local businesses, organisations and voluntary groups will come along to this event and find out more about how they can help make change happen.”
The event will be held on Monday, March 18, from 9.15am to 12.30pm at Burghley Park Golf Club, St Martins Without, Stamford, where representatives from companies and voluntary groups can learn more about Integrated Neighbourhood Working and how they can get involved.
Natalie Munslow, Lead for Integrated Neighbourhood Working in Stamford, said: “Neighbourhood working in Stamford, has seen relationships grow and develop between primary and secondary care, community care and the voluntary sector.
“The focus has been primarily with people who are frail, encouraging personalisation and self-care, but acknowledging and actioning when people require support and intervention.
“Growing relationships ensures that our population is better served by a wide and growing joined up community structure.”
Stamford GP Dan Petrie is one of those involved in Integrated Neighbourhood Working, and he added: “Increasingly neighbourhood working is bringing the right care to people at the right time allowing them to maintain their independence.
“There is a tangible increase in the feeling of a local team who are here to support our local population and I’m really proud to be a part of it.
“The plans we have for the coming year or two will only look to better this further and allow more people to live their lives the way they want and to flourish.”
